Please download forms below and fill them in before the initial consultation to help ease the process of information gathering.
Initial Consultation Form 2019
Consultations
- Consultations by appointment only.
- Cash payments and EFT only. No card services, cheques or accounts run.
- Client is responsible for claims from medical aids.
- Practice is paperless as far as possible, therefore invoice and receipts will be emailed to you at the end of the day of consultation.
- Cancellations within less than 24 hours of consultation time will be charged for. The same applies to unattended or missed consultations.
This is negotiable depending on circumstance. - Please email the three filled-in forms to my email address as stated above. Failing this, please complete and bring with to first consultation.
